Faxing Proof of Purchase

This document is for HP products used in the United States or Canada.
If you believe the warranty status on the website is incorrect, fax your warranty verification request to HP.
  1. Complete all required fields on the HP Warranty Validation form.
  2. Print the completed warranty validation form.
  3. Fax the form and proof of purchase to HP.
    • In the United States, fax the information to (866) 304-1252.
    • In Canada, fax the information to (866) 576-1765.
Once HP receives the fax, HP uses the information to determine the warranty status of your product. Within one business day, HP sends you an email regarding the outcome of your request.

Issue with my Accessories, Supplies and/or Software
Use the browse button to select the proof of purchase file from your computer. The following file types are allowed to be uploaded: JPG, PNG, DOC, DOCX, PDF, ZIP, GIF, or BMP.

When all required information is received, HP can use the information to determine the warranty status for the product that you want supported.

Tips on Scanning and Attaching Files

1. Use a device to scan or capture your document by following your product's instructions.
2. When scanning, save the file by selecting one of the following file formats: JPG, PNG, DOC, DOCX, PDF, ZIP, GIF, or BMP.
3. Make note of the location where you saved your file and ensure that it does not exceed the 5 MB size limit.
4. Use the "Browse" button within the submission form and select a file to upload in the File Open Dialog Box, and then click on the "Open" button.
5. Continue completing the form and select the "Submit" button to send your request to a HP Service Agent.
